Counseling Services

All services are discontinued effective June 30, 2010 for an undetermined length of time due to personal leave. 

    I offer mental health and substance misuse counseling services to individuals and couples who are 18 years of age and older.  I occasionally work with clients who are younger than 18 if my services are appropriate for their needs.  These instances are determined on a case-by-case basis.   The services I provide for substance misuse are appropriate for individuals who are in early or long-term recovery and are in need of relapse prevention or continuing care.  

    In addition to mental health and substance misuse counseling, I offer clinical and substance abuse/dependence diagnostic assessments (biopsychosocial assessments) that include treatment recommendations.  Diagnoses and treatment recommendations are based on the criteria established by the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR) and the American Society of Addiction Medicine's Patient Placement Criteria, Second Edition, Revised (ASAM-PPC-2R).  

    My office hours are Monday through Thursday by appointment.  I am a participating provider with a number of insurance networks (i.e., Aetna, BlueCross BlueShield, Cigna, Deer Oaks, Horizon, Humana, Lexington Group, LifeSynch, Multi-Plan, Optum, PHCS, TriCare, United Health, ValueOptions) and Employee Assistance Programs (EAP).  If I am not a network provider with your insurance carrier, my office manager will be happy to inquire about a single case agreement as well as network participation.  She will assist you in confirming your behavioral health benefits, answering any questions you may have, and in scheduling an appointment.

Other Services

Beck Anxiety Inventory, Beck Depression Inventory-II, Beck Scale for Suicide Ideation  These questionnaires are used as a tool to assist in evaluating the severity of anxiety and depressive disorders in adults and to evaluate the risk for suicide in adults. 

Brief Symptom Inventory  The BSI is for use with adults.  This is a quick form that helps evaluate for psychological problems and assists in measuring progress during and after treatment to measure changes.

Minnesota Multiphasic Personality Inventory-2 Restructured Form  The MMPI-2-RF is for use with adults.  This is an assessment of symptoms of psychopathology, personality characteristics and behavioral proclivities.

Substance Abuse Subtle Screening Inventory  I administer the SASSI-3 with biopsychosocial assessments.  I offer both the adult and juvenile versions of this questionnaire.
   The SASSI-3 questionnaire helps assess the likelihood of a substance dependence disorder.

SUBSTANCE ABUSE PROFESSIONAL - U.S. D.O.T.
 (SAP)

  I have met all the Department of Transportation (DOT) requirements to practice as a Substance Abuse Professional (SAP)  I offer SAP services to individuals who fall under DOT regulations and have violated an alcohol or other drug rule.  I realize how important it is to begin this process as soon as possible so the employee can get into an educational or treatment program and back to work as soon as possible.  Special arrangements are made, if necessary, to schedule an assessment at the earliest possible convenience.

     SUBSTANCE ABUSE EXPERT - U.S. N.R.C.
(SAE)

  I have recently met all the U.S. Nuclear Regulartory Commission's requirements to practice as a Substance Abuse Expert (SAE).  I offer SAE services to individuals who fall under NRC regulations and have violated an alcohol or other drug rule.  I realize how important it is to begin this process as soon as possible so the employee can get into an educational or treatment program and back to work as soon as possible.  Special arrangements are made, if necessary, to schedule an assessment at the earliest possible convenience.
