As of March 1, 2022, I will be offering teletherapy sessions for individuals 18 years of age and older. I will no longer be accepting insurance. If you need additional information, you may contact me by e-mail, text, or voice message.
I do not complete disability claim forms. Depending on the type of disability claim you have, your attending physician or psychiatrist will need to complete these forms based on their evaluation of your situation.

These services are billed separately and payment is due at the time they are administered. Due to the transition to teletherapy, some of these will no longer be available except during in-office appointments.
Beck Anxiety Inventory, Beck Depression Inventory-II, Beck Scale for Suicide Ideation These questionnaires are used as a tool to assist in evaluating the severity of anxiety and depressive disorders in adults and to evaluate the risk for suicide in adults.
Brief Symptom Inventory The BSI is for use with adults. This is a quick form that helps evaluate for psychological problems and assists in measuring progress during and after treatment to measure changes.
Minnesota Multiphasic Personality Inventory-2 Restructured Form The MMPI-2-RF is for use with adults. This is an assessment of symptoms of psychopathology, personality characteristics and behavioral proclivities.
Substance Abuse Subtle Screening Inventory I administer the SASSI-3 with biopsychosocial assessments. I offer both the adult and juvenile versions of this questionnaire. The SASSI-3 questionnaire helps assess the likelihood of a substance dependence disorder.
